PLEASE NOTE CAREFULLY: As of April 2002, Jamaican passports
are being issued by the Passport Office in Kingston. Applicants should therefore
read all instructions carefully to ensure that applications are correctly
completed. Doing so will help to avoid delays in processing.
The following are instructions for the completion of the Jamaican passport application form. Please pay close attention to the following:

Ensure the applicant's signature is affixed in the CENTER of the signature box at the top of page 1.

Married men and women are to complete Section B.

Indicate the document number and date of issue of your citizenship document in Section C.

Complete Section D for children under 18 years old. The parent or legal guardian's signature must be placed beside the minor's signature on the line in Section E. The parent giving consent MUST be on the minor's birth certificate. The parent or legal guardian is required to submit a state issued picture identification.

Tick the appropriate box in Section E and affix your signature to the signature line.
- Section F must be completed with the name and relationship for your Emergency contacts.
- Section G must be signed by either a Notary Public, Jamaican Honorary Consul or Jamaican Diplomatic/Consular Officer.

Persons who are required to wear headgear for religious purposes, must complete Section H.
Renewal of Passport/Damaged Passport (Adults):
Applicants must submit the following documents:
- Completed passport application form
- Two (2) identical non digitized colour photographs one of which should be certified by the Notary Public who signs Section G
- Expired Passport
- Original Birth Certificate
- Original Marriage Certificate, where applicable
- Original Death Certificate, where applicable
- Original Adoption Register, where applicable
- Divorce Judgement, where applicable
If born abroad, in addition to the Birth Certificate, please submit one of the following:
Certificate of Naturalization
Letter of Certificate of Citizenship
Certificate of Registration

All first time applicants must visit the Consulate General to be interviewed by a Consular Officer and submit the following documents:
Completed passport application form
Two (2) identical non digitized colour photographs one of which should be certified by the Notary Public who signs Section G
Original Birth Certificate
Original Marriage Certificate, where applicable
Original Adoption Register, where applicable
Divorce Judgement, if applicable
If Born abroad, in addition to the Birth Certificate, please submit one of the following:
Certificate of Naturalization
Letter of Certificate of Citizenship
Certificate of Registration
In addition, ONE of the following supplementary documents must be presented:

National Voter's Identification Card
Work Permit bearing a photograph from the country of residence

First- Time Applicant (Minors):

Mailed Applications:
Section G and one (1) photograph must be certified by a Notary Public. Their stamp should be affixed in the area provided. These persons need not be Jamaicans or Commonwealth Citizens.
NB: Photocopies of documents, even if notarized, will not be accepted. All original documents will be kept and returned at the end of Passport processing period.
Processing of mailed applications starts from the date of approval by a Consular Officer and not from the date application was received.

The Consulate General of Jamaica accepts only Money Orders
and Cash (Do not send cash in the mail). Personal
checks and credit cards are not accepted.
The processing
time for Passports is approximately four to six (4- 6) weeks from the date of
approval.
Applicants are advised not to make any travel plans prior to receiving your passport.
